Course Change Request Form

This form should be used by pre-college participants interested in making changes to a semester schedule. This includes: dropping a course, adding a course, or changing the grade option for a course. 

This form must be submitted by the deadlines indicated in your Canvas student orientation course. Refunds (if applicable) are determined by the date received. If requested changes result in a balance due, this must be paid in full before a change can be processed.

Please note that all requests for continued enrollment are subject to approval by the Arts & Sciences Pre-College Programs office and are contingent upon meeting program expectations and maintaining satisfactory academic performance.

 

Include course name, department name and course number (ie. Matrix Algebra MATH 3300)
